HOW TO ACCESS SERVICES
Individuals, family members, or agencies serving individuals with special needs may contact VITO to make a Referral for Services.

ISPs/PCPs
Each individual will be assessed to determine eligibility for a VITO Service.  Once accepted into the Program, the Individual’s strengths, needs, and preferences will be identified through Person-Centered Planning so that an Individual Service Plan (ISP) or Person-Centered Plan (PCP) can be developed.  The ISP/PCP will include identified outcomes so that the individual can live more independently.
