IN HOME SERVICES
- 1:1 Training in the Individual’s home and/or in the community
- Training focuses on vital Daily Living Skills, Self-Help Skills, Age-Appropriate Recreation, Adaptive Behaviors, Communication Skills, Community Integration, and Social Skills Development.
- Goal is independence to the fullest extent possible in all facets of life.

HOW TO ACCESS SERVICES Individuals, family members, or agencies serving individuals with special needs may contact VITO to make a Referral for Services.
ISPs/PCPs Each individual will be assessed to determine eligibility for a VITO Service. Once accepted into the Program, the Individual’s strengths, needs, and preferences will be identified through Person-Centered Planning so that an Individual Service Plan (ISP) or Person-Centered Plan (PCP) can be developed. The ISP/PCP will include identified outcomes so that the individual can live more independently.
